If you're paying by credit card, we recommend using our online registration instead of filling out the paper form, it's safe, secure, faster and it does all the math calculations for you! When registering online, you do not need to mail in the paper version of our registration form as that information is already included in the online process. Please review our cancellation and refund policies carefully.
If you are registering a child with Autism or Aspergers, you must use the Integrated ASD Integrated Camp Registration Form found below on this page.

Health Form

We require a new updated Health Form each year for all campers, along with a current wallet-sized photo.
Important: it is crucial that we receive the Health Form information at least 4 weeks BEFORE your child arrives at camp so that our Health Staff can review the information and our kitchen staff can accommodate food allergies accordingly in the menu preparations. Even if your child does not have any concerns, we need the emergency contact information in order to prepare the cabin sheet for your child's counsellor. Your child will not be permitted to attend camp without this form on file prior to camp.

Anaphylaxis Emergency Form

If your child has a life-threatening allergy, please be sure to complete and forward an Anaphylaxis Emergency Form along with your child's Health Form

Activity Choices (2-week & 3-week sessions only)

completed forms due by May 15th so that we may schedule campers. Failure to submit this form in a timely manner will mean your child may not get their preferred choices.
